Medicare Plans — How to enroll (2012)

For Essential (PDP) and Prime (PDP) plans

Enrolling in our PDP plans is easy. First, check our frequently asked questions to see if you're eligible. Then, just choose the enrollment option that's most convenient for you:

Drop in Come to our headquarters at 6450 US Highway 1 in Rockledge, Monday through Friday from 8 a.m. to 5 p.m. to talk with us face-to-face.

Call us to schedule your appointment with a Medicare Advantage Consultant and enroll in the comfort of your home (1-855-737-4347, TTY/TDD 1-800-955-8771, weekdays 8 am to 8 pm and Saturday 8 am to noon).

Call Medicare You can also call 1-800-MEDICARE 24 hours a day, 7 days a week, and enroll over the phone. Be sure to tell them which plan you would like from Health First Medicare Plans.

How do I pay my plan premium?

You have three options to pay your plan premium. You can indicate which option you choose on the enrollment form or short enrollment form:

  1. Electronic funds transfer (EFT). Your bank can automatically transfer your premium to us. 
  2. Monthly bill. We can send you a bill and you can send us a check. If you choose to pay by check please remember to include it with your enrollment or short enrollment form.
  3. Have it withheld from your Social Security benefit check. (Note: If you decide to have your plan premium withheld it from your Social Security benefit check or move from withhold to direct bill, it could take up to three months for it to take effect and you will be ultimately be held responsible for those premiums.)
